what is the proper procedure to serve a summons to a bank to appear or produce records?

I have judgment *($19000) against employer for writing a NSF payroll check under a canceled llc. Data from sec of state shows former employer is the only person listed on cancelled llc. how whould i properly request data from bank to show employer had sole account control. Employer intentionly Wrote/submitted check and not seeking to remedy NSF is fraud
