There are no "forms" to do an adoption. Your chance of doing one pro se is not much better than your chance of winning a top lottery prize. Even most lawyers lack the skills to do an adoption, and most lawyers who adopt retain an adoption/family specialist. Almost every type of pro se case goes badly, and almost any other type case is less technical and complex than adoption law. In other words, stop looking for forms that do not exist, and get a good lawyer. It's absolutely necessary.
Bear in mind that as part of a grandparent adoption you will need the proper consent of both parties, or persuade a judge to terminate rights of both parents.
